A cardboard box with Isaac's name scribbled on the side, and a drawing of a stick figure, presumably one that is Isaac. When picked up, Isaac wears a box around his torso with said name scribbled on the side.

Variants of items (such as Troll Bombs, Nickels or Eternal Hearts ) may also spawn instead.
Bugs
- Regular or half hearts never drop, only Soul Hearts.
Notes
- If all other trinkets were already dropped before taking the Box, it will drop an extra tarot card instead of a trinket.
